HERBALISM.....

Herbalism, also known as herbal medicine, herbology, and phytotherapy, is a folk and traditional medicinal practice based on the use of plants and plant extracts.

Utilizing the healing properties of plants is an ancient practice. People in all continents have long used hundreds, if not thousands, of indigenous plants for treatment of various ailments dating back to prehistory. These plants are still widely used in ethnomedicine around the world.

The first generally accepted use of plants as healing agents were depicted in the cave paintings discovered in the Lascaux caves in France, which have been Radiocarbon dated to between 13,000 - 25,000 BCE.

Anthropologists theorize that over time, and with trial and error, a small base of knowledge would have been acquired within early tribal communities. As this knowledge base expanded over the generations, the specialized role of the herbalist emerged. The process would likely have occurred in varying manners within a wide diversity of cultures.

Plants have an almost limitless ability to synthesize aromatic substances, most of which are phenols or their oxygen-substituted derivatives such as tannins. Most are secondary metabolites, of which at least 12,000 have been isolated, a number estimated to be less than 10% of the total. In many cases, these substances (esp. alkaloids) serve as plant defense mechanisms against predation by microorganisms, insects, and herbivores. Many of the herbs and spices used by humans to season food yield useful medicinal compounds.

The use of and search for drugs and dietary supplements derived from plants have accelerated in recent years. Pharmacologists, microbiologists, botanists, and natural-products chemists are combing the Earth for phytochemicals and leads that could be developed for treatment of various diseases. In fact, many modern drugs have been derived from plants.

The use of herbs to treat disease is almost universal among non-industrialized societies. A number of traditions came to dominate the practice of herbal medicine in the Western world at the end of the twentieth century:

The Western, based on Greek and Roman sources,
The Ayurvedic from India, and
Chinese herbal medicine (Chinese herbology).
Many of the pharmaceuticals currently available to Western physicians have a long history of use as herbal remedies, including opium, aspirin, digitalis, and quinine.

[edit] Biological background
All plants produce chemical compounds as part of their normal metabolic activities. These can be split into primary metabolites, such as sugars and fats, found in all plants, and secondary metabolites found in a smaller range of plants, some only in a particular genus or species.

The autologous functions of secondary metabolites are varied. For example, as toxins to deter predation, or to attract insects for pollination. It is these secondary metabolites which can have therapeutic actions in humans and which can be refined to produce drugs. Some examples are inulin from the roots of dahlias, quinine from the cinchona, morphine and codeine from the poppy, and digoxin from the foxglove, and salicylic acid alpha-hydroxybenzoic acid, C6H4(OH)(COOH). S.A. led to the development of Aspirin, acetyl-salicylic acid, originally a trade name, patented by Bayer.

In 2004 the National Center for Complementary and Alternative Medicine began funding clinical trials into the effectiveness of herbal medicine.[1]

[edit] Popularity
A survey released in May 2004[4] by the National Center for Complementary and Alternative Medicine focused on who used complementary and alternative medicines (CAM), what was used, and why it was used. The survey was limited to adults age 18 years and over during 2002 living in the United States. According to this survey, herbal therapy, or use of natural products other than vitamins and minerals, was the most commonly used CAM therapy (18.9%)[5] when all use of prayer was excluded.

Herbal remedies are most common in Europe. In Germany, the term apothecary (Apotheke) is still used, and next to prescription drugs one can order essential oils, herbal extracts, or herbal teas. It is even seen as a preferred treatment over the unnecessary overuse of industrialized production of chemical medication.

In the United Kingdom, the training of medical herbalists is undertaken in private colleges. Recently, Bachelor of Science degrees in herbal medicine are offered at Universities such as Middlesex University, University of Central Lancashire, University of Westminster and Napier University in Edinburgh.

At UCLAN the training of medical herbalists is extensive, and involves the study of anatomy, physiology, biochemistry, pathology, differential diagnosis etc. This enables the herbalist to be able to talk on equal terms with conventional medical practitioners. They learn about when they can treat, and when they should refer. They study the biochemistry of the body and the chemistry and structure of plants. Alongside this, they study traditional herbal medicine: Indications and Actions, and Energetics. They learn to recognise where conventional medical opinion diverges from traditional herbal medicine doctrine. They look at lots of different models for health care. A medical herbalist is trained to view a situation from multiple perspectives in a non-dogmatic way.


[edit] Types of herbal medicine
Medicinal plants can be used by anyone, for example as part of a salad, an herbal tea or supplement, although some herbs considered dangerous are restricted from sale to the public. Sometimes such herbs are provided to professional herbalists by specialist companies. Many herbalists, both professional and amateur, often grow or wildcraft their own herbs. Many common weeds have medicinal properties (e.g. dandelion).

Medicinal herbs can be used in various forms:

There are two methods of making herbal teas, infusion and decoction. Infusion is steeping lighter parts of the plant (leaves, flowers, light stems) in boiled water for several minutes. Decoction is boiling tougher parts, such as roots or bark for a longer period of time. Herbal teas are often used as a home remedy, and as an alternative to tea and coffee.

Some popular herbal teas include borage, chamomile, dandelion, elderflower, hibiscus, nettle, and various species of mint. Each herb has unique medicinal properties, and a range of secondary effects; and this is linked to its use as a casual drink. For instance, Borage can be used medicinally (amongst other things) as an aid against depression; and it may be drunk more casually to lift the spirits. It was traditionally served as a pep drink for weary travellers. Secondary effects include use as an anti-inflammatory or balsam; and hormonal and metabolic regulation. Other herbs may have similar side-effects, but in different proportions and are used in different ways.

Mixing Herbs. To counteract the various complications and side-effects of an ailment, or to produce a more rounded taste, a number of herbs may be mixed, and formulas are the preferred method of giving herbs by professional herbalists. A well-known mixture used against a cold includes eucalyptus leaf, mint leaf (which contains Menthol) and juniper berry. Another is the age-old favourite "dandelion and burdock", from which the popular fizzy drink was derived.

Fresh or Dried? Many flower and leaf herbs lose volatile compounds within a few hours, as the juices and oils evaporate, the scent leaks away, and the chemicals change their form. Drying concentrates other compounds as water is removed. Most herbal traditions use dried material and the reported effects for each herb tend to be based upon dried herbs unless otherwise specified.

If you are using fresh herbs, you will need more of them, and the tea will have a somewhat different effect. Finely chop the leaf immediately before using it.

Generic Western Tea is usually the leaf of one specific plant, Camelia Sinensis, which grows mainly in India and China. It can be seen as just one of many herbal teas. It is a stimulant, and its main property is to increase alertness, along with a slightly sedative or calming effect. But unlike a fresh herbal tea, it may include artificial additives to enhance the taste and to preserve it in the shops. More expensive recipes include Darjeeling (from Darjeeling in India) and Earl Grey (which has bergamot added).

Infusion Methods. Some simple methods include -

Drop the herb directly into the water, steep and drink, including the chopped leaves.
Use a tea strainer which fits over the top of a cup.
Use a tea strainer which can be lowered into a teapot.
Cut the top off a normal teabag and empty it; replace the contents with your herb; fold over the top and staple it closed - then use it as normal teabag.
With all of these methods, let stand, covered, for at least 15 minutes to get medicinal benefits.


[edit] Coffee
Coffee is prepared by roasting and then grinding up the ingredients, or sometimes by decoction. It is normally stored as a powder or as granules. This is dropped directly into the hot water. Some preparations dissolve completely, while others remain partly solid, giving the drink a gritty texture.

Where the coffee remains gritty, it must be left to infuse before drinking. To avoid the gritty texture, a machine such as a coffee percolator or French press may be used.

Generic Western Coffee is the roasted seeds of either Coffea arabica or Coffea canephora. Its properties are similar to the generic tea plant Camelia Sinensis, but it has about twice as much caffeine, a richer, mellower taste, and different additives. The roasting process eliminates caffeine, and (like tea) the darker the coffee, the less caffeine it contains. When coffee is scarce, people often drink coffee substitutes.

Herbal teas include many of the coffee substitutes, and others; but they are used here for a wide range of properties, rather than merely to simulate the taste of coffee.

[edit] Herbal tinctures
Steeping a medicinal plant in alcohol extracts the alcohol-soluble principles into a liquid form that can be stored for long periods. Different concentrations of alcohol are used to extract different constituents of the plants. For example; resins require high alcohol content and sugars usually require low alcohol content for optimal extraction.

There are many schools of thought about tincture making. In the traditional view an herb is either steeped once (single maceration) or more than once. In a double maceration the mark (or used plant material) is removed and replaced by a new batch (using the same alcohol) thus increasing the strength of the tincture. Sometimes the mark is then ashed (burnt until ash) and added back in which increase the amount of some minerals in the tincture.

In the scientific model tincture strengths are measured by a ratio of herb to alcohol (1:5 and 1:2 are the most common where the 1:2 is the stronger tincture). Many tinctures use a combination of vegetable glycerine and alcohol to extract which changes the compounds that are extracted.

Herbalists often mix several herbal tinctures to form an individualized prescription for each patient.

Plant tinctures are also the basis for many homeopathic medicines. Herbal medicine and homeopathic medicine should not be confused with one another as they are very different. (see homeopathy)

[edit] Fluid extracts
Fluid extracts are stronger than herbal tinctures, and can be preserved with alcohol or glycerin. They are just highly concentrated tinctures, made by distilling off some of the alcohol used in the tincture process. The final result is a liquid plant compound that can be 40 times more potent than a tincture.

Note: glycerates are herbal extracts that use glycerin as the sole extractant. They are very different and often have completely different medicinal properties than alcohol extracts. Tinctures or fluid extracts that are alcohol free should have the alcohol removed after the extraction process and replaced with glycerin which then acts just as the preservative.

[edit] Solid extracts
Solid extracts are made from tinctures just like fluid extracts, but the entire solvent is separated from the plant compound, leaving a soft paste-like solid exract or a dry solid extract that is often as much as 400 times more potent than tinctures. Solid extracts can be diluted back to either fluid extracts or tinctures.

Many solid extracts are made in the way that apple butter is made, by simply cooking the plant material and water slowly over low heat until it forms a paste. Sometimes these are sold with a preservative added (glycerine is the most common), and sometimes they need to be refrigerated when they are opened (like apple butter).

[edit] Herbal poultices
Poultices are a solid, vegetable fat-based mixture used externally. They have the shortest life span of any herbal remedy and must be made fresh for every use.

Poultices can be made with water or just fresh ground herb. They are applied topically often in conjunction with a heat source (hot water bottle or heating pad). They are used mostly (but not exclusively) for a localized injury (sprains, strains, scrapes, burns, bruises or cuts). They are excellent for hiking injuries (where there are usually plants and not pharmacies), but can be used for many other things.

[edit] Powdered herbs and tablets
Herbs that are dried and (usually) certain parts are separated out then ground to a powder fine consistency. Powered matter can then be compressed or put in an empty capsule to form a tablet. Most tablets these days are made from some form of concentrated extract and not just plain herbal powders. This method is optimal for foul tasting herbs, highly concentrated forms and for travelling.

Powders are also used simply in their powdered form. Many Chinese medicine practitioners use powdered herbs as a base for making teas. Powdered herbs are also used topically as powders where dry skin is advantageous (some diaper rashes respond bettered to powdered herbs than ointments).


[edit] Herbal ointments
An ointment or salve is a semi-solid preparation made to be applied to the skin. Depending on the purpose for which it is designed and its method of preparation, the texture can vary from very greasy to a thick paste.

Most ointments use vegetable oils (olive and almond are common) in conjunction with a thickener like bees wax. The herbs are often extracted in the oils for months before they are ready to be made into an ointment. The herb is then removed from the oil and it is slowly heated with bees wax to form the desired consistency. Vitamin E and essential oils are often used to preserve the salve or ointment.

The simplest ointments use petroleum jelly as a base. In traditional ointments, a combination of carrier oils is used that helps them to be absorbed through the skin, plus hardening agents to create the desired texture.

Extraction of volatile liquid plant materials and other aromatic compounds from plants gives essential oils. These plant oils may be used internally in some forms of herbal medicine as well as in aromatherapy and generally for their perfume, although their medicinal use as a natural treatment (alternative medicine) has proved highly efficacious in the treatment of headache and muscle pain[6], joint pain[7] and certain skin diseases[8]


[edit] Herbal supplements
Herbal supplements tend to be commercial products in tablet or capsule form manufactured and marketed by the health food industry for sale in retail outlets to the general public, although there are some types that are sold only to healthcare practitioners for prescription.

Herbal supplements are often standardized to contain stated levels of active phytochemicals. Standardization can be done in a number of different ways. Many companies extract the supposedly active constituents and add them back into a base like rice fiber. This is no longer an herbal preparation as the whole herb is no longer present. Some companies follow the same procedure but add the extracted constituent to whole powdered herb. Some companies simply concentrate their whole herb until the desired constituent concentration is reached.

Many herbalists do not agree with the extraction/standardization of active ingredients, preferring instead to use the whole plant.

There are hundreds of herbal remedies. An experienced practitioner can offer a comprehensive holistic approach to health. Examples of some commonly used herbal medicines:

Artichoke and several other plants reduced total serum cholesterol levels in preliminary studies.[9]
Black cohosh and other plants that contain phytoestrogens (plant molecules with estrogen activity) have some benefits for treatment of symptoms resulting from menopause.[10]
Echinacea extracts limit the length of colds in some clinical trials, although some studies have found it to have no effect at lower dosages than are normally given by herbalists.[11]
Garlic lowers total cholesterol levels, mildly reduces blood pressure, reduces platelet aggregation, and has antibacterial properties.[12] It also has strong anti-viral and anti-fungal properties.
Grapefruit seed extract as a natural antimicrobial has minimal effectiveness as an anti-bacterial, anti-parasitic, and anti-fungal herb.[13][14][15][16][17]
Nigella sativa (Black cumin) is a general medicinal plant used for diverse ailments such as cough, pulmonary infections, asthma, influenza, allergy, hypertension and stomach ache. The seeds are considered carminative, stimulant, diuretic and galactogogue. It is often taken with honey. Seed powder or oil is externally applied for eruptions of skin.
Peppermint tea for problems with the digestive tract, including irritable bowel syndrome and nausea.
Rauvolfia Serpentina, used extensively in India for sleeplessness, anxiety, and high blood pressure. The first proven allopathic medicine for high blood pressure was extracted from this herb.
St. John's wort, has yielded positive results, proving more effective than a placebo for the treatment of mild to moderate depression in some clinical trials.[18]
Valerian root can be used to treat insomnia. [2]
Lemon juice or apple cider vinegar can be used to treat acne.[citation needed]
Green tea can heal scars faster.[citation needed]
Lemon grass can lower cholesterol [citation needed]
Honey can be a solution for cholesterol.[citation needed][3]

[edit] Risks and benefits
A common misconception about herbalism and the use of 'natural' products in general, is that 'natural' equals safe. However many plants have chemical defence mechanisms against predators that can have adverse or lethal effects on humans. Examples are poison hemlock and nightshade, which can be deadly, although they are not sold as herbs. Herbs can also have undesirable side-effects just as pharmaceutical products can. These problems are exacerbated by lack of control over dosage and purity. Furthermore, if given in conjunction with drugs, there is danger of 'summation', where the herb and the drug have similar actions and add together to make an 'overdose'. In animals, there are other dangers. There may be residues in food from farm animals (e.g. eggs, milk, meat) or danger of 'doping' in competition animals. The latter may also apply to human athletes.

There is a danger that herbal remedies will be used in place of other medical treatments which have been scientifically tested for safety and efficacy, resulting in the development or worsening of a medical condition which could have been better prevented or treated. There is also a danger that an herbal remedy may itself cause harm which is unanticipated due to a lack of a full understanding of its composition and biochemical effects.

However, if a qualified Medical Herbalist, is consulted, he or she will have the required knowledge to prescribe and dispense herbs tailored to the individual, taking into account their medical history and any other medication. A qualified Herbalist will have studied for four years and passed exams in botanical and medical subjects, and will be a member of one of the profession's governing bodies. These include The College of Phytotherapy Practitioners and The National Institute of Medical Herbalists.

Forthcoming changes to laws regulating Herbal products in the UK, will ensure the quality of herbal products used. Most Medical Herbalists will be using herbal products from a supplier who already meets these standards.


[edit] Effectiveness
As noted above, there have been scientific studies which show that certain plant products can cure or prevent certain diseases. The gold standard for pharmaceutical testing is repeated, large-scale, randomized, double-blind tests. Some plant products or pharmaceutical drugs derived from them are incorporated into mainstream medicine. To recoup the considerable costs of testing to the regulatory standards, the substances are patented by pharmaceutical companies and sold for high profit. Pharmaceutical firms argue that the regulations protect public safety. Cynics point out that they have a secondary effect of setting a high financial barrier to competition.

Most herbal traditions have accumulated knowledge without modern scientific controls to distinguish between the placebo effect, the body's natural ability to heal itself, and the actual benefits of the herbs themselves. Many herbs have shown positive results in in-vitro, animal model or small-scale clinical tests. The few randomized, double-blind tests that receive attention in mainstream medical publications are often questioned on methodological grounds or interpretation. Studies tend to carry more weight if they are performed in the same country as the medical scientists evaluating them. Likewise, studies published in wide circulation magazines such as JAMA receive more consideration than those published in specialized herbal journals. Fortunately there is now much scientific research into the action and effectiveness of herbal remedies and many are justifying their traditional use. High quality trials involving proper controls and double blind methods are being increasingly carried out, with many useful results.

Herbalists tend to use parts of plants, such as the roots or leaves but not isolate particular phytochemicals. They argue that the synergy of the combined substances enhances the efficacy and dilutes toxicity. Western medicine on the other hand prefers single ingredients on the grounds that dosage can be more easily quantified. Dosage is in general an outstanding issue for herbal treatments: while most conventional medicines are heavily tested to determine the most effective and safest dosages (especially in relation to things like body weight, drug interactions, etc.), there are few established dosage standards for various herbal treatments on the market. Furthermore, herbal medicines taken in whole form cannot generally guarantee a consistent dosage or drug quality (since certain samples may contain more or less of a given active ingredient).

The issue of regulation is an area of continuing controversy in the EU and USA. On one end of the spectrum, some herbalists and consumers maintain that traditional remedies that have a long history of use do not require the level of safety testing as xenobiotics or single ingredients in an artificially concentrated form. On the other hand, others are in favor of legally enforced quality standards, safety testing and prescription by a qualified practitioner. Some professional herbalist organizations have made statements calling for a category of regulation for herbal products. Yet others agree with the need for more quality testing but believe it can be managed through reputation without government intervention. Clearly the full debate involves issues of politics, economics and the role of government that are beyond the scope of this article.


[edit] Name confusion
The common names of herbs (folk taxonomy) may not reflect differences in scientific taxonomy, and the same (or a very similar) common name might group together different plant species with different effects. For example, in 1993 in Belgium, in a formula created by medical doctors including someTCM herbs for weight loss, one herb (Stephania tetrandra) was swapped for another (Aristolochia fangchi) whose name in Chinese was extremely similar but which contained higher levels of a renal toxin, aristolochic acid; this quid pro quo resulted in 105 cases of kidney damage. Note that neither herb used in a TCMcontext would be used for weight loss or given for long periods of time. In Chinese medicine these herbs are used for certain forms of acute arthritis and edema. [edit] Standards and quality control
The legal status of herbal ingredients varies by country. For example, Ayurvedic herbal products may contain levels of heavy metals that are considered unsafe in the U.S., but heavy metals are considered therapeutic in Ayurvedic medicine.

In the United States, most herbal remedies are regulated as dietary supplements. Many herbs for home use could also be grown in a small home garden.

In the UK, herbal remedies that are bought over the counter are regualted as supplements,as in the US. However, herbal remedies prescribed and dispensed by a qualified Medical Herbalist, after a personal consultation, are regualted as medicines. A Medical Herbalist can prescribe some herbs which are not available over the counter, covered by Schedule III of the Medicines Act, for example.


[edit] Medical interaction
In consultation with a physician, usage of herbal remedies should be clarified, as some herbal remedies have the potential to cause adverse drug interactions when used in combination with various prescription and over-the-counter pharmaceuticals. Dangerously low blood pressure may result from the combination of an herbal remedy that lowers blood pressure together with prescription medicine that has the same effect. In particular, many herbs should be avoided during pregnancy.[22] However, most herbal books alert the reader to necessary precautions.

Not all physicians may be familiar with the effects of different types of herbal medicine, but general practitioners should be able to refer patients to a specialist, or investigate the medical literature on their behalf. A qualified Medical Herbalist, however, will be familiar with most commonly used medical drugs and their modes of action and therefore will only prescribe herbal remedies which will not interfere with these.

Momordica Charantia (ampalaya)

Ampalaya is a vegetable grown throughout the Philippines. It is mostly cultivated, although wild forms can be found. It grows wild in the remote areas of Mt. Banahaw. As the English name suggests (bitter melon), the melon has a bitter taste due to the presence of momordicin. There has been much research done on the effectiveness of using Momordica Charantia in the treatment of diabetes.

It has been shown to increase production of beta cells by the pancreas, thereby improving the body’s ability to produce insulin. It has been recommended by the Department of Health of the Philippines, as one of the best herbal medicines for it's ability to help with liver problems, Diabetes and HIV.

It is a common herb used in Chinese herbology. In the Philippines, the leaves are often used for children's coughs. It is also used in the treatment of skin diseases, sterility in women, as a parasiticide, as an antipyretic, and as a purgative.

Called guayaba in Spanish-speaking countries and goiaba in Brazil, guava is a common shade tree or shrub in door-yard gardens in the tropics. It provides shade while the guava fruits are eaten fresh and made into drinks, ice cream, and preserves. In the richness of the Amazon, guava fruits often grow well beyond the size of tennis balls on well-branched trees or shrubs reaching up to 20 m high. Cultivated varieties average about 10 meters in height and produce lemon-sized fruits. The tree is easily identified by its distinctive thin, smooth, copper-colored bark that flakes off, showing a greenish layer beneath.

Guava fruit today is considered minor in terms of commercial world trade but is widely grown in the tropics, enriching the diet of hundreds of millions of people in the tropics of the world. Guava has spread widely throughout the tropics because it thrives in a variety of soils, propagates easily, and bears fruit relatively quickly. The fruits contain numerous seeds that can produce a mature fruit-bearing plant within four years. In the Amazon rainforest guava fruits are much enjoyed by birds and monkeys, which disperse guava seeds in their droppings and cause spontaneous clumps of guava trees to grow throughout the rainforest.

TRIBAL AND HERBAL MEDICINE USES

Guava may have been domesticated in Peru several thousand years ago; Peruvian archaeological sites have revealed guava seeds found stored with beans, corn, squash, and other cultivated plants. Guava fruit is still enjoyed as a sweet treat by indigenous peoples throughout the rainforest, and the leaves and bark of the guava tree have a long history of medicinal uses that are still employed today.

The Tikuna Indians decoct the leaves or bark of guava as a cure for diarrhea. In fact, an infusion or decoction made from the leaves and/or bark has been used by many tribes for diarrhea and dysentery throughout the Amazon, and Indians also employ it for sore throats, vomiting, stomach upsets, for vertigo, and to regulate menstrual periods. Tender leaves are chewed for bleeding gums and bad breath, and it is said to prevent hangovers (if chewed before drinking). Indians throughout the Amazon gargle a leaf decoction for mouth sores, bleeding gums, or use it as a douche for vaginal discharge and to tighten and tone vaginal walls after childbirth. A decoction of the bark and/or leaves or a flower infusion is used topically for wounds, ulcers and skin sores. Flowers are also mashed and applied to painful eye conditions such as sun strain, conjunctivitis or eye injuries.

Centuries ago, European adventurers, traders, and missionaries in the Amazon Basin took the much enjoyed and tasty fruits to Africa, Asia, India, and the Pacific tropical regions, so that it is now cultivated throughout the tropical regions of the world. Commercially the fruit is consumed fresh or used in the making of jams, jellies, paste or hardened jam, and juice. Guava leaves are in the Dutch Pharmacopoeia for the treatment of diarrhea, and the leaves are still used for diarrhea in Latin America, Central and West Africa, and Southeast Asia. In Peruvian herbal medicine systems today the plant is employed for diarrhea, gastroenteritis, intestinal worms, gastric disorders, vomiting, coughs, vaginal discharges, menstrual pain and hemorrhages, and edema. In Brazil guava is considered an astringent drying agent and diuretic and is used for the same conditions as in Peru. A decoction is also recommended as a gargle for sore throats, laryngitis and swelling of the mouth, and used externally for skin ulcers, and vaginal irritation and discharges.

PLANT CHEMICALS

Guava is rich in tannins, phenols, triterpenes, flavonoids, essential oils, saponins, carotenoids, lectins, vitamins, fiber and fatty acids. Guava fruit is higher in vitamin C than citrus (80 mg of vitamin C in 100 g of fruit) and contains appreciable amounts of vitamin A as well. Guava fruits are also a good source of pectin - a dietary fiber. The leaves of guava are rich in flavonoids, in particular, quercetin. Much of guava's therapeutic activity is attributed to these flavonoids. The flavonoids have demonstrated antibacterial activity. Quercetin is thought to contribute to the anti-diarrhea effect of guava; it is able to relax intestinal smooth muscle and inhibit bowel contractions. In addition, other flavonoids and triterpenes in guava leaves show antispasmodic activity. Guava also has antioxidant properties which is attributed to the polyphenols found in the leaves.

BIOLOGICAL ACTIVITIES AND CLINICAL RESEARCH

The long history of guava's use has led modern-day researchers to study guava extracts. Its traditional use for diarrhea, gastroenteritis and other digestive complaints has been validated in numerous clinical studies. A plant drug has even been developed from guava leaves (standardized to its quercetin content) for the treatment of acute diarrhea. Human clinical trials with the drug indicate its effectiveness in treating diarrhea in adults. Guava leaf extracts and fruit juice has also been clinically studied for infantile diarrhea. In a clinical study with 62 infants with infantile rotaviral enteritis, the recovery rate was 3 days (87.1%) in those treated with guava, and diarrhea ceased in a shorter time period than controls. It was concluded in the study that guava has "good curative effect on infantile rotaviral enteritis."

Guava has many different properties that contribute to its antidiarrheal effect: it has been documented with pronounced antibacterial, antiamebic and antispasmodic activity. It has also shown to have a tranquilizing effect on intestinal smooth muscle, inhibit chemical processes found in diarrhea and aid in the re-absorption of water in the intestines. In other research, an alcoholic leaf extract was reported to have a morphine-like effect, by inhibiting the gastrointestinal release of chemicals in acute diarrheal disease. This morphine-like effect was thought to be related to the chemical quercetin. In addition, lectin chemicals in guava were shown to bind to E-coli (a common diarrhea-causing organism), preventing its adhesion to the intestinal wall and thus preventing infection (and resulting diarrhea).

The effective use of guava in diarrhea, dysentery and gastroenteritis can also be related to guava's documented antibacterial properties. Bark and leaf extracts have shown to have in vitro toxic action against numerous bacteria. In several studies guava showed significant antibacterial activity against such common diarrhea-causing bacteria as Staphylococcus, Shigella, Salmonella, Bacillus, E. coli, Clostridium, and Pseudomonas. It has also demonstrated antifungal, anti-yeast (candida), anti-amebic, and antimalarial actions.

In a recent study with guinea pigs (in 2003) Brazilian researchers reported that guava leaf extracts have numerous effects on the cardiovascular system which might be beneficial in treating irregular heat beat (arrhythmia). Previous research indicated guava leaf provided antioxidant effects beneficial to the heart, heart protective properties, and improved myocardial function. In two randomized human studies, the consumption of guava fruit for 12 weeks was shown to reduce blood pressure by an average 8 points, decrease total cholesterol levels by 9%, decrease triglycerides by almost 8%, and increase "good" HDL cholesterol by 8%. The effects were attributed to the high potassium and soluble fiber content of the fruit (however 1-2 pounds of fruit was consumed daily by the study subjects to obtain these results!). In other animal studies guava leaf extracts have evidenced analgesic, sedative, and central nervous system (CNS) depressant activity, as well as a cough suppressant actions. The fruit or fruit juice has been documented to lower blood sugar levels in normal and diabetic animals and humans. Most of these studies confirm the plant's many uses in tropical herbal medicine systems.

CURRENT PRACTICAL USES

Guava, known as the poor man's apple of the tropics, has a long history of traditional use, much of which is being validated by scientific research. It is a wonderful natural remedy for diarrhea - safe enough even for young children. For infants and children under the age of 2, just a cup daily of guava fruit juice is helpful for diarrhea. For older children and adults, a cup once or twice daily of a leaf decoction is the tropical herbal medicine standard. Though not widely available in the U.S. market, tea-cut and powdered leaves can be obtained from larger health food stores or suppliers of bulk botanicals. Newer in the market are guava leaf extracts that are used in various herbal formulas for a myriad of purposes; from herbal antibiotic and diarrhea formulas to bowel health and weight loss formulas. Toxicity studies with rats and mice, as well as controlled human studies show both the leaf and fruit to be safe and without side effects.

Traditional Preparation: The fruit and juice is freely consumed for its great taste, nutritional benefit and nutrient content, as well as an effective children's diarrhea remedy. The leaves are prepared in a standard decoction and dosages are generally 1 cup 1-3 times daily.

Contraindications:

Guava has recently demonstrated cardiac depressant activity and should be used with caution by those on heart medications.
Guava fruit has shown to lower blood sugar levels and it should be avoided by people with hypoglycemia.

Drug Interactions: None reported, however excessive or chronic consumption of guava may potentiate some heart medications.

Lavender and Tea Tree Oils May Cause Breast Growth in Boys

A study published in this week’s issue of the New England Journal of Medicine suggests that repeated topical use of products containing lavender oil and/or tea tree oil may cause prepubertal gynecomastia, a rare condition resulting in enlarged breast tissue in boys prior to puberty, and for which a cause is seldom identified.

Researchers at the National Institute of Environmental Health Sciences (NIEHS), part of the National Institutes of Health (NIH), confirmed in laboratory studies what a pediatric endocrinologist at the University of Colorado at Denver and Health Science Center’s School of Medicine suspected after diagnosing three of his young male patients with prepubertal gynecomastia.

The researchers found an association between the use of products containing these oils and the rare disorder, but cautioned more research is needed. At this point, the findings are only applicable to young males with unexplainable enlarged breasts who are regularly using products containing these essential oils.

"We want to encourage doctors who may be seeing patients with gynecomastia to ask their patients about the products they are using. Patients with prepubertal gynecomastia may want to consider reducing the use of products that contain these oils,” said Ken Korach, Ph.D., chief, Laboratory Reproductive and Developmental Toxicology at NIEHS and author on the study.

“Although we found an association between exposure to these essential oils and gynecomastia, further research is needed to determine the prevalence of prepubertal gynecomastia in boys using products containing lavender and tea tree oils. Results of such epidemiological studies are important to tell us how strong the association is between topical application of the oils and prepubertal gynecomastia”

The three otherwise healthy Caucasian boys, ages four, seven and 10 years, had normal hormonal levels when they were diagnosed with gynecomastia by Clifford Bloch, M.D., in Colorado. All had either used lavender-scented soap and skin lotions, or shampoos or styling products that contained tea tree oil and lavender oil as ingredients. In each case, several months after the suspected products were discontinued, the gynecomastia had subsided or resolved.

After Bloch discussed the cases with Korach, the NIEHS researchers conducted experiments using human cells to determine if the oils mimic the effects of estrogen, the female hormone that stimulates breast tissue growth, or inhibited the effects of androgen, the hormone known to control masculine characteristics and inhibit the growth of breast tissue. The researchers tested the ability of the oils to modulate or inhibit gene expression.

“The results of our laboratory studies confirm that pure lavender and tea tree oils can mimic the actions of estrogens and inhibit the effects of androgens,” said Korach. “This combinatorial activity makes them somewhat unique as endocrine disruptors.”

Bloch said the laboratory studies support his hypothesis. “Since there was no identifiable cause for prepubertal gynecomastia in the three patients we reported, we speculated that environmental factors might be contributing to their condition. Together, the case histories and NIEHS in vitro studies provide support for our hypothesis that topical exposure to lavender and tea tree oils likely caused gynecomastia in the three patients.”

The oils did not alter the levels of the usual forms of circulating estrogens and androgens in the boys. “We do not anticipate any long term effects on hormonal levels,” said Derek Henley, Ph.D., the lead NIEHS author on the study. It is unknown whether the oils have similar endocrine disrupting effects in prepubertal girls, adolescents or adults.

“This study clearly demonstrates how clinical observations can be supported by basic science research,” said NIEHS Director David A. Schwartz, M.D.

These essential oils might now be considered endocrine disruptors since they appeared to have caused an imbalance in estrogen and androgen signaling. Endocrine disruptors are naturally occurring compounds or synthetic chemicals that may interfere with the production or activity of hormones of the endocrine system leading to adverse health effects.

Taking the Hype Out of Hypnotherapy

It can ease pain, anxiety and addiction, but beware of false claims

Forget those movie images of Svengali-like hypnotists waving pocket watches.
Today's hypnotherapy is practiced by qualified physicians and has long been recognized by leading medical organizations -- including the American Medical Association, the American Psychological Association and the U.S. National Institutes of Health -- as an adjunct therapy useful in easing a range of ailments.

"It's a tool we use in our clinical work -- regardless of whether you're a dentist or psychologist or physician," said Marc Oster, a clinical psychologist and hypnotherapist based in Arlington Heights, Ill.
Dr. David Spiegel, an expert on hypnotherapy and a professor of psychiatry and behavioral sciences at Stanford University in California, agreed.
"Hypnotherapy is just a form of highly focused attention, and there are therapeutic strategies that you employ using that highly focused attention," he explained.

For that reason, both experts stressed that patients who want to try hypnosis as a treatment tool should consult a practitioner licensed in some other form of medicine -- an M.D., a psychologist or a dentist, for example.

Their reasoning: "If you don't have clinical training, how can you work with clinical problems?" Oster said. While hypnosis can help people stop smoking, for example, a mere hypnotist may not be able to spot and treat underlying problems.

"Maybe the person is using smoking to help manage their anxiety," Oster said. "So then you're not treating the problem, just a symptom." In the case of psychiatric woes, especially, poorly guided hypnotherapy may even worsen the situation, experts say.

What is hypnotherapy? According to Oster, patients are usually "talked" into a state of highly focused, suggestible attentiveness where they are able to clear away mental "clutter" and focus on whatever problem it is that concerns them. In most cases, practitioners teach patients self-hypnosis techniques they can use at home.

Patients do not relinquish self-control, Oster said.

"Actually, from a clinical perspective, that's the opposite of what we do with people," he explained. "People come to see us to develop greater willpower and have more self-control, more confidence in themselves. You don't help that by taking it away."

Using electroencephalogram [EEG] and other methods, science is beginning to determine what happens to the hypnotized brain. "We're getting to the point where we can see that the hypnotic brain looks different from the resting or sleeping brain," Oster said. Hypnotized individuals are usually physically at ease, with lowered blood pressure and heart rates, while feeling fully awake and mentally attentive.

Studies have shown hypnosis can be a useful adjunct therapy against many ills, including:

Gastrointestinal problems. "For irritable bowel syndrome, especially, hypnosis has been demonstrated to be about 80 percent effective in reducing or eliminating symptoms. Medicine cannot do that," Oster said.

Pain. "It's been clearly helpful there for hundreds of years," Spiegel said. In many cases, patients with chronic pain use self-hypnosis techniques to "turn down" pain, like lowering the volume on a radio. Spiegel said patients can also use the technique to help get through invasive or painful medical procedures, such as dentistry or even cardiac catheterization.

Smoking and other addictions. "Half of people will typically stop smoking after a single [hypnosis] session, and half of those won't have a cigarette for two years," Spiegel said. In the world of smoking-cessation, a 25 percent long-term success rate is considered impressive, he said.

Weight loss. "There's some pretty good research that says hypnosis is helpful," Oster said. "It seems to help people stay focused on their goals."

Finding an effective, qualified hypnotherapist is easy if one consults one of two recognized associations: the practice-oriented American Society of Clinical Hypnosis, and a more research-oriented group, the Society for Clinical and Experimental Hypnosis. Both groups mandate that hypnotherapists also be licensed in some form of clinical training.

Oster said consumers should be wary of claims that seem exaggerated or too good to be true. "If someone says their success rate with smoking is 90 to 95 percent, for example, I'd stay away," he said.

Both experts stressed that hypnotherapy is really directed by the patient, anyway, not the practitioner.

"It's a collaborative relationship between two people," Oster said, "you and I. It's something I do with you, not to you." A good hypnotherapist simply teaches techniques that allows a patient to fulfill his or her goals, he said.

"Patients look at it as, 'I'm doing this -- I'm learning to help myself,' " he said.

AIDS virus weakness detected



Scientists have captured an image of the AIDS virus in a biological handshake with the body's immune cells it attacks, and said on they hope this can help lead to a better vaccine against the disease.

They pinpointed a place on the outside of HIV that could be vulnerable to antibodies that could block it from infecting human cells.

U.S. National Institutes of Health researcher Peter Kwong said the study, published in the journal Nature, may reveal HIV's long-sought "site of vulnerability" that can be targeted with a vaccine aimed at preventing initial infection.

"Having that site and knowing that you can make antibodies against it means that a vaccine is possible," Kwong said in a telephone interview.

"It doesn't say we've gotten there. But it's taken it off the list from an impossible dream and converted it to something that is a (mere) technical barrier."

Experts agree that a vaccine is the only hope of stopping the pandemic of AIDS, which has killed more than 25 million people since it was first recognized in 1981. About 40 million people now live with HIV, with sub-Saharan Africa hardest hit.

But while dozens of potential vaccines are in development, only two AIDS vaccine candidates are in advanced human trials -- one made by Merck and Co. and another by Sanofi-Aventis SA.
Because the virus attacks immune system cells, it has been especially difficult to design a vaccine to fight it.

The team at the National Institute of Allergy and Infectious Diseases, part of the NIH, made atomic-level images of the virus.

They revealed the structure of a protein on the surface of HIV as it looks while the protein is bound to an infection-fighting antibody. They said this protein, called gp120, seems susceptible to attack by this antibody, which is called b12 and is capable of broadly neutralizing the virus.
An antibody is a protein produced by the immune system in response to invading microorganisms, such as viruses and bacteria.

CAUTIOUS HANDSHAKE

The researchers detailed the precise interaction that occurs as the virus tries to grab and infect protective immune system cells.

"The first contact is like a cautious handshake, which then becomes a hearty bear hug," said Dr. Gary Nabel, an NIH vaccine expert and a co-author of the research.

The virus uses the protein gp120 to gain entry into the CD4 T-cells it infects. But the researchers also found that the antibody b12 can block this process.

The virus mutates quickly and continuously to beat the immune system's efforts to target it. It also is cloaked in such a way that it stops antibodies from blocking the proteins that HIV uses to bind to a cell and infect it.

So this is a critical area of vulnerability, Nabel said. "This is certainly one of the best leads to come along in recent years," he said.

NIAID Director Dr. Anthony Fauci said the findings are of great importance, but much more work in animal and human studies is needed, and any vaccine is years away.

"I don't think there's any one particular thing that, in and of itself, is the show-stopper. But I don't think we could really make substantial, fundamentally scientifically based progress until we got this very important information," he said.

Guided imagery can boost the body's healing powers


What if your mind's eye could take you to a place so peaceful that the experience eased your pain or sped your recovery from surgery? It's not such a far-fetched concept.

Guided imagery," a type of mind-body therapy that uses visualized images to communicate to the housekeeping systems of the body, is making its way into traditional medical settings.

"People are just now taking a very serious look at it," said David E. Bresler, co-founder of the Academy for Guided Imagery, in Malibu, Calif., and author of the book Free Yourself From Pain. "There are a handful of hospitals around the country and around the world that are starting to implement these programs," he said

In one study, researchers at Harvard Medical School found that more than 30 percent of U.S. adults have used some form of mind-body medicine, a category that includes imagery, according to the National Center for Complementary and Alternative Medicine.

Bresler, a traditionally trained Ph.D. neuroscientist, first became intrigued with alternative methods of pain relief in the early 1970s, as founder and director of the University of California, Los Angeles, Pain Control Unit. Patients often used vivid images to describe their pain. It felt like an ice pick to one person, fire ants to another. One particular patient, a psychiatrist with a painful rectal carcinoma, suffered low back pain that he said "felt like a dog chewing on my spine."

Bresler knew that when patients used their imagination to go to a peaceful place, it helped them to relax, so he guided the agitated psychiatrist through a relaxation exercise. When the man's pain flared up, Bresler instructed him to speak to the dog. Would it let go of his spine? Then, an astonishing thing happened -- when the dog let go to talk, the man's pain subsided.

Today, guided imagery has numerous applications. Sports psychologists use it to enhance athletes' physical performance. Cancer centers often use it to relieve patients' pain and nausea.

In a 2004 study in the journal Pain, researchers at Cincinnati Children's Hospital Medical Center found that children who used guided-imagery tapes before and after routine surgery had significantly less pain and anxiety than a control group. More recently, researchers examined how children used these tapes, which suggested that they "go" to a park, at least in their mind. Many, though, put their own spin on the proposed image, allowing them to escape to places like a swimming pool, a lake or an amusement park.

Bresler said imagery is the language of the autonomic nervous system, the part of the nervous system that regulates many involuntary body functions, such as heart rate, blood pressure and digestion. "So, when you're working with images, it's really a set of instructions to the system," he said.

Victoria Menzies, an imagery researcher and professor at Florida International University's School of Nursing, in Miami, said the first step for a patient in any guided-imagery session is to develop a rapport with the guide who is asking you to close your eyes and relax. Once the patient is in a relaxed state, the guide will either offer an image or ask the person to come up with their own, someplace where he or she would feel calm and safe or joyful -- the mountains, the seacoast, a favorite room in their home, whatever.

Engaging the senses is the next step, she explained. The guide might ask where you are and what you see, hear, smell, feel and even taste.

Menzies led a 10-week guided imagery intervention for a small group of patients with fibromyalgia, a condition involving chronic pain and fatigue. In the study, published in January 2006 in the Journal of Alternative and Complementary Medicine, one group of patients received usual care and used a set of guided-imagery audiotapes. The other group received only usual care. Compared with the controls, the patients who participated in guided imagery were better able to perform activities of daily living and had a greater sense of being able to manage their pain and other symptoms, the study showed.

What's more, Menzies found, "The pain did not change, but the ability to cope with the pain was improved."

Bresler considers it shocking that medical colleagues would reach their hands into someone's body and remove organs before allowing a patient to go through an imaging exercise.

"It only takes a few moments to do these things and to really check on the wisdom from inside, because there is tremendous wisdom that's being generated if only we'd listen to it," he said.

Antioxidant Activity of Tea Unaffected by Milk

The antioxidant activity of green and black tea (Camellia sinensis (L.) Kuntze, Theaceae) in the body (in vivo) is well established, but an important question remains: Does the addition of milk to tea inhibit the bioavailability of antioxidant tea polyphenols? Not according to the results of this Dutch study, which showed that a single dose of either black or green tea with or without milk caused a significant rise in plasma antioxidant activity.

The crossover study compared the antioxidant effects of green tea, black tea, and non-carbonated mineral water with or without milk in 21 healthy volunteers. Each participant received a dose of one of the six test substances on six different days. A single dose of tea was defined as 2 g of tea solids in 300 ml of water (Lipton Research Blend, Lipton, Englewood Cliffs, NJ). The researchers utilized the ferric reducing ability of plasma (FRAP) assay to measure both plasma antioxidant and catechin levels. Blood samples were taken before consumption of the test substances and again 30, 60, 90, and 120 minutes after consumption. According to the results, both green and black tea caused a significant rise in plasma antioxidant and catechin levels, but the effect of green tea was significantly greater at all time points. The addition of milk to either type of tea did not significantly alter responses.

While a limited number of studies support these results, others have shown that milk had a negative impact on the antioxidant capacity of tea. An earlier study published in the European Journal of Clinical Nutrition concluded that while the addition of milk to tea had no effect on antioxidant activity in vitro, it did appear to interfere with absorption of tea polyphenols in vivo. The authors of the older study offered two possible explanations for this effect. First, because milk proteins can cause complexation (binding) of tea polyphenols, the researchers proposed that milk/polyphenol complexes resist gastric breakdown, rendering the polyphenols unavailable for absorption. They also theorized that milk might hinder polyphenol absorption by increasing gastric pH

On the other hand, the authors of the more recent study suggested that the antioxidant assay utilized by Serafini and colleagues (called the Total Radical trapping Ability of Plasma, or TRAP assay) might be less reliable than the FRAP method, as TRAP may be associated with a higher degree of variability. - Evelyn Leigh, HRF

Oolong tea helps in the treatment of stubborn atopic dermatitis

An open Japanese study suggests that consumption of oolong tea (Camellia sinensis) helps speed clearance of recalcitrant atopic dermatitis lesions.

The 118 study participants continued their usual dermatologic treatments but also drank oolong tea (10 g steeped in 1000 mL water a day, divided into three doses).

Beneficial results were noted after one to two weeks, and 74 (63%) of the participants showed marked to moderate improvement of lesions after one month.

After 6 months, 64 patients (54%) still demonstrated a good response to treatment.

The study builds on animal research showing that oral administration of green, black, or oolong tea suppressed allergic skin reactions. Uehara M, Sugiura J, Sakurai K.

No colchicine in ginkgo: Independent testing refutes results of flawed research

Independent analyses of raw powdered Ginkgo biloba and ginkgo extract have discredited a recent study suggesting that ginkgo supplements contain the toxic alkaloid colchicine. The flawed study, "Identification of Colchicine in Placental Blood from Patients Using Herbal Medicines," by Petty et al., was released on the American Chemical Society's website on August 4, 2001, but apparently has not yet been formally published. The authors of the study warn that consumption of ginkgo supplements by pregnant women may pose dangers to developing fetuses, based on an analysis that they believe revealed the presence of colchicine in ginkgo.

Major media sources have publicized the study's conclusions, but herb experts and other scientists have strongly criticized the validity of the conclusions and the research methods used by the researchers. Some have suggested that the investigators misidentified a non-toxic ginkgo compound as colchicine. This ginkgo compound reportedly has a structure similar to that of colchicine, and the researchers failed to perform a test necessary to differentiate between the two. "Ginkgo simply does not contain colchicine," said HRF President Rob McCaleb. "These scientists should have done their homework before submitting their erroneous conclusions for publication."

Ginkgo has been the subject of hundreds of clinical, pharmacological, chemical, and toxicological studies, not a single one of which has identified colchicine as a gingko constituent. "For the authors to say 'such supplements should be avoided by pregnant women or those trying to conceive because the colchicine in them could affect the viability of a fetus,' as quoted in Chemical and Engineering News, is an unacceptable leap, give the questions raised about this report, and its conflict with the existing scientific literature with colchicine," said John Cordaro, president and chief executive officer of Council for Responsible Nutrition (CRN). Both CRN and the American Herbal Products Association (AHPA) commissioned third party analyses of finished ginkgo products sold in the US. Colchicine has not been detected in any of the products.

In addition, a comprehensive search of the scientific literature on ginkgo conducted by Dr. Norman Farnsworth, Distinguished Research Professor of Pharmacognosy at the University of Illinois at Chicago, further confirmed that colchicine has never been detected in ginkgo or ginkgo products. "Anyone who thinks that colchicine can be found naturally in ginkgo is not qualified to be a peer reviewer of this paper," Farnsworth said. -Evelyn Leigh

Acupuncture: an effective treatment for tension headaches

Acupuncture is an effective treatment for tension headaches, say German researchers from the Centre for Complementary Medicine Research at Technische Universitat in Munich The ancient Chinese therapy cut the rates of headaches by nearly half in a study of 270 people.

"A significant proportion of patients with tension-type headaches benefited from acupuncture" said Dr Wolfgang Weidenhammer, one of the research team. Patients were given 12 sessions of acupuncture over 8 weeks.

The headache rate in patients given the traditional treatment dropped by almost half.

They experienced 7 fewer days of headaches in the four weeks after treatment had finished.

The results of the research, published in the British Medical Journal (29 July 2005) stated that "acupuncture was well tolerated and improvements lasted several months after completion of treatment"
